Includes: Monthly supply of testosterone, weekly vitamin injections performed in-office.
Gold $200/month
Includes: Monthly supply of testosterone, weekly vitamin injections, and direct primary care services.

Membership Payments are taken out on the 5th or the 15th of the month.

Additional charges for medications supplements, labs, ect are charged at the time of service.

Cancelation
Need to cancel membership – No Problem. Notify the office 10 days before the next pay cycle. Please note re-enrollment is based on provider availability.

Can I use my health insurance to cover my testosterone replacement?
Most certainly, but Reform ABQ will only bill insurance for testosterone replacement if you choose to have a provider from our office as your primary care provider and if we must be in-network with your health plan. It would be best if you also had a testosterone level that health insurance deems appropriate to start testosterone replacement therapy.
What level will qualify me for my health insurance to cover testosterone replacement?
Typically, health insurance requires a man’s testosterone level to be at or below 350ng/dL on two separate blood draws.
What are the benefits of a testosterone replacement membership?
When you join as a TRT member, you get the following benefits: • In-house medication dispensing. • Our trained staff will provide all injections in-office if you are uncomfortable injecting at home. • Weekly, customized vitamin injections. • Erectile dysfunction and estrogen-blocking medications are included in the price. • In-house lab draws and therapeutic blood draws if indicated.
What benefits do I lose if I bill my health insurance for testosterone replacement?
If you choose to use your health insurance for testosterone replacement, you will have to do the following: •Your Prescription will be sent to an outside pharmacy for pick-up. •You will have to inject your testosterone at home (if injectable forms are being used). • You are responsible for all drug costs. • We will see you in-office every month to three months (at the provider's discretion) to allow for medication refills. You will be responsible for co-payments, deductibles, or coinsurances. • You will not receive vitamin injections. • You will have your labs drawn at an outside lab, such as Quest or Tricore. • If you need to donate blood, you will be responsible for locating a local blood bank to donate to.
Can I use my health insurance for Primary Care and pay for a TRT membership?
Yes! But remember that all your primary care needs will require a separate visit from your injection appointment, and all co-payments, deductibles, or coinsurances are billed according to your health plan.
